All letters should be addressed to New York Foundation for the Arts, but mailed to Mad-G Productions, LLC. (Do not address donor letters to Film Fatale, Inc. or Mad-G Productions.) The contribution letter is needed from individuals, corporations and small foundations that do not have their own grant agreement letter they send with their grants. This letter must be from the donor addressed to NYFA and include the following information: Please find enclosed a contribution for the New York Foundation for the Arts in the amount of_____________________. It is my wish this contribution be used to support _____________________ project name / /organization. (Ex. “Player Hating” by Maggie Hadleigh-West / Mad-G Productions, LLC. The donor will need to sign the Contribution Letter and include their address for the acknowledgment letter from NYFA. All checks must be made out to NYFA, but sent to Film Fatale Inc.
